In this inpatient palliative care project, we aimed to enhance patient outcomes and satisfaction through collaborative efforts. By focusing on key goals such as reducing readmissions and length of stay, we integrated the palliative care assessment tool and discussions into the treatment process, emphasizing code status and the creation of POLST documents for suitable patient conditions. Research indicates that both inpatient and outpatient palliative services can significantly improve patient experiences when a structured treatment plan is followed. This initiative underscores the importance of holistic care in achieving better health outcomes.
